Setting environment variables varies depending on OS. For PostgreSQL installed on Ubuntu or Debian via apt-postgresql, the preferred way is to edit <filename>/etc/postgresql/<replaceable>10</replaceable>/<replaceable>main</replaceable>/environment</filename> where 10 refers to version of PostgreSQL and main refers to the cluster.

Core postgis extension includes geometry, geography, spatial_ref_sys and all the functions and comments. Raster and topology are packaged as a separate extension.
